He noticed a shackle being fitted with an ordinary screw instead a proper bolt with nut and pin. He stopped the work, took the shackle and brought it to HSE. The equipment was quarantined until it’s bolt will be found.
He informed the toolpusher about a wash out at the drill pipe box, which he noticed while he was performing the stand connection.
He noticed that the fire extinguishers in the diesel tank area are positioned too close to the tank and in case of a fire it can’t be safely reached.
He noticed the crew was about to use one improper lifting sling for unloading some BOP components. Stopped them and sent them to the storage container to look for the proper cable.
He intervened when he noticed that one colleague was too close to the equipment which was lifted. He warned his colleague to keep distance and he complied.
